STEP 1
Complete the CDC Dog Import Form
First, to bring your dog into the USA, you must fill out the CDC Dog Import Form. This form is mandatory for all dogs entering from Canada.
- Who Fills Out the Form? The person bringing the dog into the U.S.
- What If I'm Brining More Than 1 Dog? Each dog must have its own form.
- When Do I Complete This Form? Fill out the form on the day of travel or up to six months before.
- How Do I Present The Form? Show the form's receipt to U.S. customs officials or airlines, either printed or on your phone.

Important Items To Note
In addition to the CDC Dog Import Form, there are 3 other requirements that pet parents must meet to ensure a smooth entry into the USA from Canada:
- Health Status: Dogs must appear healthy upon arrival.
- Age Requirement: Dogs must be at least 6 months old at the time of entry or return to the U.S.
- Microchip: Dogs must have a microchip that can be detected with a universal scanner.
